Observation

0335 (2)- Child restraint was not limited to holding a child gently and was not performed by a trained staff person. 0335 (4) – Staff did not remove herself from the situation on 1/7/2021, even after the child’s body got stiff and they “hit the floor”. Staff let go of child after she heard a “crunch” noise” and child “grabbed his leg”. Reports collected through LD/CPS investigation show that staff person had a history of interactions with child as noted on a progress report on 1/5/2021 she noted “I snapped and ripped his coloring book in half”. Leadership failed offer support to this staff person. 0335 (5) – Physical restraint was used several times on the child and each time the center failed to notify the department on the following dates; 10/8/2020, 12/22/2020, 1/5/2021,. The center failed to develop a written plan to reduce the need for future physical restraint.
